What Is the Effect of Chocolate Cyst on Ovarian Reserve?

In infertile women who have a chocolate cyst, the ovarian reserve is in quite a low state. It is thought that ovarian damage due to endometriosis surgery affects the ovarian reserve through a different mechanism.

In this study published by the journal “Assisted Reproduction and Genetics”, researchers working at the University of Arizona and the Brigham and Women’s Hospital examined 671 people who came for “In Vitro Fertilisation” treatment between April 2015 and May 2018. The people included in this study were divided into three different groups according to their cases. The first group was separated as “women with endometriosis who had previously had ovarian surgery”, the second group as “women with endometriosis who had not previously had ovarian surgery” and the third group as “women without endometriosis who could not become pregnant because of male infertility”, and the cases were collected.

After the cases were divided into groups, the blood anti-müllerian hormone (AMH) and follicle stimulating hormone (FSH) levels/ratios of these people were determined and examined. With this examination it was determined that the average AMH level of women with endometriosis who had or had not had ovarian surgery was at a lower level than in women who could not become pregnant because of male infertility. When compared with people who had male factor infertility, more women with endometriosis had an AMH level lower than 1 ng/ml.

In this research, when a comparison was made with women who had male factor infertility, it was determined that the FSH levels of women with infertility due to endometriosis were higher. AMH is known as the hormone secreted by developing ovarian cells. A low AMH level, which correlates with a low ovarian reserve, is a good indicator of ovarian reserve.
